Dear Sir/Madam,
I have made application for the insurance of my clinic to your company in month of Jan 2011. I have yet not received any response from your company in this regard.
I have made application in the duly filled form and also submitted necessary documents required for the insurance. I am still confused about the delay in your response.
So I hereby appeal you to look into the matter and please convey me the reasons of such late. If there is lack of some documents which is required for the insurance then I am ready to submit the same as per the requirement.
I would appreciate you earlier response to my appeal so that I can take necessary steps in this regard. I hope that you will take my appeal in your due consideration and will take prompt action in this regard.
