Dear Mr. / Ms. (Last Name), Subject: (Your Account Number)

My credit report for the period (date) provided by (Name of Credit Reporting Agency) showed credit inquiries coming from your company. I do not recall giving you any authorization to conduct such inquiries on my credit standing nor have I entered into any transactions with you necessitating any credit inquiries to be performed by your company.

I have stated my case and I demand from you that the unauthorized credit inquiries reported by your company be deleted from my credit report. I have strived to keep my credit report free from derogatory items and your inquiries will be posing harm for my future loan applications.

Failure to comply with my request will mean defiance for the consumer protection stipulated in the Fair Credit Reporting Act and I will be complied to take the necessary legal action against you to protect my right.

I will wait for your reply on this and I demand a written proof that the unauthorized inquiries have been deleted from my credit report.

Sincerely,

(Your Signature) (Your Name in Print)
