Dr. Andrew Moore,

Knee Surgeons of Alabama,

839 Hutchings,

4th Avenue Jones,

Dear Sir,

Future Insurance Agency is undertaking a research on Bright Bernard, a client of yours who is yet to acquire a health policy from our firm. We would like any information requiring his recent knee surgery and plan to factor in what we have in his previous records with what you have on him. This will give us better insight and our agents a clear judgment on the condition of his bone structure and any other information that you may deem necessary to give. We have his full consent and plan to make sure that he gets full compensation in case of any risky event.

Yours truly,

Deborah Knowles
